There are no refunds for the program.

INFORMED CONSENT

This program has three phases, each requiring 7 days of using imagery and other mental exercises after you have completed a brief questionnaire. These exercises will take a relatively brief period of time each day. No other physical or mental activity is required, although you will be given information to read as well as individualized reports as feedback from your questionnaire responses. It may be, although remote, that for some of you the imagery and other exercises may arouse uncomfortable feelings. You are free to withdraw at any time. In the event of any adverse effects, discontinue the program at once and contact Power-Imagery Partners at elizabeth@powerimagery.com. The benefit of participation for you is that an individualized program of mental imagery will be designed for you based on your responses to a brief data collection tool. All information submitted by you will be kept in confidence by Power-Imagery Partners. This information will be stored in a computer file that can be accessed only by Power-Imagery Partners through the use of special passwords. The Power-Imagery Partners will retain the information since it is their ethical obligation to safeguard confidentiality in order to protect your rights as a consumer. The information provided to us will be used only by Power-Imagery Partners and will not be disclosed, disseminated, nor distributed, sold or bartered under any circumstances, and will remain strictly within the confines of Power-Imagery Partners.
